Indications
The FOB test (acronym for Fecal Occult Blood) is used to detect microscopic traces of blood in the stool that are not visible to the naked eye. It is indicated for the preventive screening and monitoring of gastrointestinal disorders.
The test is Recommended:
- Every 1–2 years after age 45–50 as a prevention for colon cancer.
- If you have symptoms: visible or invisible blood in the stool, anemia, abdominal pain, unexplained weight loss.
- If you have a family history of intestinal tumors.

There are two main types of fecal occult blood tests:
- Immunochemical test (ifobt or Fit) – the most modern:
- Detects human hemoglobin (specific for blood from the colon or rectum).
- No special diet required before the test.
- 1–2 samples are sufficient.
- Results in 10 minutes.
- Guaiac test – the oldest:
- Reacts with the iron in hemoglobin.
- Can false positives may occur for red meat, certain medications, or vegetables.
- Requires more careful dietary considerations before the test.
The procedure for the at-home immunochemical test involves:
- Collect a stool sample with the appropriate swab/wand.
- Insert it into the reagent vial.
- Shake and place a few drops in the well of the test cassette.
- Wait 5–10 minutes.

It is recommended to perform the test on a normal diet, preferably with the immunochemical test (ifobt), as it is more effective. Reliable and simple.
Warnings
The test has limitations:
- It does not identify the cause of the blood, which could be due to: hemorrhoids, fissures, colitis, polyps, or tumors.
- If the result is positive, it is mandatory to consult your doctor for a colonoscopy or more in-depth tests.
If you are following a national screening program, do not replace the official test with a do-it-yourself test.
