Excipients
Sodium carboxymethylcellulose, glycerol, methyl parahydroxybenzoate, propyl parahydroxybenzoate, sodium benzoate, purified water.
Therapeutic indications
Lidofast 2.5% urethral gel Endourethral maneuvers (introduction of catheters, dilations, cystoscopies, etc.) combining the lubricating and anesthetic action. Lidofast 1% gel Esophagoscopic intubations and for curative anesthesia, tracheobronchoscopy, pharyngoscopy, gastroscopy, proctoscopy and all exploratory and curative endoscopic procedures. The product, combining lubricating and anesthetic action, helps prevent spastic reactions and reflexes during endoscopic procedures that originate from the mucous membranes with which the instruments come into contact.
Contraindications
Known hypersensitivity to amide-type local anesthetics and other components of the product. Generally contraindicated during pregnancy and breastfeeding (see also section 4.6).
Dosage
Lidofast 2.5% urethral gel. Push the gel directly into the urethra by squeezing the tube and simultaneously massaging the posterior urethra, applying urethral forceps for 10 minutes, after which the desired endourethral maneuvers can be performed. The contents of the tube are sufficient to completely fill the urethra. The introduction of the gel into the urethra is facilitated by the use of the attached plastic applicator. Lidofast 1% gel. Carefully spread a layer of product on the instrument before inserting it. The neutrality of the vehicle of both product formulations, its complete solubility in water and the absence of fatty substances make it suitable for use in both formulations. that, even with continued use of Lidofast, the lenses of the instruments used for the various types of maneuvers do not fog up and their rubber parts do not deteriorate.
Warnings and Precautions
The efficacy and safety of lidocaine depend on correct dosage and application technique. It is therefore necessary to use the smallest amount of the preparation sufficient to obtain the desired effect, applying it with caution to subjects with severely damaged mucous membranes or sites of inflammatory processes and sepsis that could cause excessive absorption of the active ingredient. Excessive absorption of lidocaine can cause side effects on the central nervous system and the cardiovascular system, especially in children, the elderly, and debilitated subjects. Therefore, great caution is required when using Lidofast in these subjects. However, application for very prolonged periods should be avoided. Products for topical application, especially if used for a prolonged period or repeatedly, can give rise to sensitization phenomena. In this case, it is necessary to suspend treatment and institute appropriate therapy. The product contains parahydroxybenzoates, which can cause allergic reactions, generally delayed, such as contact dermatitis, and more rarely, immediate reactions with urticaria and bronchospasm. The product contains sodium benzoate, which is a mild irritant to the skin, eyes, and mucous membranes. It may also increase the risk of jaundice in newborns.
Interactions
Propanolol and cimetidine may reduce the hepatic metabolism of lidocaine, increasing its plasma concentrations or the half-life of the elimination phase.
Undesirable effects
Locally, hypersensitivity reactions characterized by pain, burning, and itching may occur. Systemic reactions are generally rare, however, hypersensitivity reactions up to anaphylactic shock may occur.
Pregnancy and breastfeeding
Studies in rats and rabbits have not revealed any risks to the fetus. Safety has not been established in humans. This should be borne in mind before administering lidocaine to pregnant women, particularly during the early stages. Since lidocaine is distributed in breast milk, topical preparations, especially those applied to the tracheobronchial mucosa, should be used with caution in breastfeeding women.
